The primary aim of this service is to provide community and primary care practitioners with an accessible, specialist multidisciplinary team experienced in the management of chronic pain with the ability to access specialist secondary care management if necessary.
All initial assessments are undertaken by our Pain Assessment Team (PAT) to determine which treatment pathway best suits. The PAT team provides confirmation and explanation of diagnosis and negotiates a suitable treatment plan with the patient, which may be organised through secondary care, the GP or continued within the team.
The team can further explain investigation results and deal with the impact the long term condition may be having on the individual, and liaise with the appropriate services to facilitate recovery. Patients are provided with self help information when appropriate and signposted to our specialist therapy and Cognitive Behavioral Therapy (CBT) teams to undertake more structured self-management programmes with the overall aim of reducing chronicity and disablement and promote independence and self-management.
